Accreditation Acceptance Guidelines

You are required to:

  • Complete the Insurance Schedule as detailed in the Guidelines for Completion of Insurance Schedule;
  • Read the Safety and Health Overview Statement for Service Providers. Ask questions for understanding if required;
  • Read the Environment Overview Statement for Service Providers. Ask questions for understanding if required.

If you accept the responsibility to :

  • Provide accurate information;
  • Advise Verve Energy immediately of any change of insurer, level of nature of cover; and renewal of insurance; and
  • Abide by the principles and expectations contained in the Overview Statements;

then

  • Complete the name, company and date details; and
  • Sign.

For accreditation, return with the completed Supplier Request Form and Service Supplier Accreditation Insurance Schedule.
