Please download and complete the Insurance Schedule which forms part of the Service Accreditation Package. Instructions are provided below to help you complete the Schedule.
Guidelines for Completion of Service Supplier Accreditation Insurance Schedule
To complete the Insurance Schedule:
EITHER
a) Have your insurance provider / broker complete as detailed below and sign;
OR
b) Obtain Certificates of Currency for each insurance type from your insurance provider / broker and then personally complete as below and sign.
Information is to be provided for the insurance types listed on the schedule:
a) Workers' Compensation Insurance, including cover for directors, officers and principals;
b) Employer's Liability Insurance if applicable (applicable in the eastern states only);
c) Public and Product Liability Insurance; and
d) Insurance in respect of any motor vehicles used in carrying out the Services (including third party personal injury and property damage insurance).
For each Insurance type list:
a) Name of Insurer;
b) Policy Number;
c) Verve Energy noted on policy. This shows that Principal's Indemnity (a requirement for accreditation) has been added to the policy. Principal's Indemnity can be requested from your insurer / broker and generally comes at no charge. Circle either Y or N;
d) Date that the policy commenced - Effect Date;
e) Date that the policy will end - Expiry Date;
f) Value of cover contained in the policy;
g) Determine if the amount of cover meets the minimum requirement (amount listed on the Insurance Schedule) for that type of insurance (a requirement of accreditation). Circle either Y or N.
For Workers' Compensation Insurance also identify if cover for Working Directors, Officers and Principals is included. Circle either Y or N (as appropriate) and if Y, provide all names.
Please note that this information will be required on an annual basis triggered by the expiry date of the insurance policy/ies.
